An AmeriHealth Caritas Louisiana case manager received a referral from the Rapid Response Team (RRT) for case management outreach in Baton Rouge, LA.
The member initially called RRT because of a barrier to transportation. An in-depth assessment uncovered the member's diabetes compliance issues. Her glucometer didn't work, plus she lacked adequate eye care and couldn't afford her $140 eyeglasses prescription. Jodi, a registered nurse, contacted the primary care physician's office and obtained a prescription for the glucometer. She also contacted a local Lion's Club chapter to provide assistance with the cost of the glasses.
The member told AmeriHealth Caritas Louisiana, "This managed Medicaid program with an RN is the best thing Louisiana could have ever done."
