Yearly Medicaid Redetermination
If you are a member of a Medicaid plan with AmeriHealth Caritas, it’s time for your annual redetermination — the yearly process to ensure that you keep your health care benefits. Please complete and return your eligibility review form as soon as you receive it.

Care coordination
Our Care Managers will team with you and your providers to create a treatment plan that works just for you. Together, we will keep track of your health, remind you of appointments, and even set up rides to appointments if you need them.
Community outreach
So much of your health depends on issues that aren't physical. We work with local groups to address these social issues that impact your health, like homelessness, domestic violence, unemployment, and education.
Removing barriers to health care
We, along with your providers, help address any cultural or language barriers to your care. For example, through Cultural and Linguistic Appropriate Services (CLAS) standards, we offer low-cost, 24/7 access to phone and video interpretation services.
